Care Transitions Resource Specialist
Salary Grade: 8 ($ 29,432 to $$ 41,204.80)
Hours: 40 per week
Location: Lincoln, Maine
The purpose of this position is to help people understand their problems, provide information to make informed decisions and about possible solutions to facilitate independent living for older adults and their families. The Care Transitions Resource Specialist provides accurate and unbiased information about a variety of services available through local agencies and programs; makes referrals to specific service agencies or programs; may advocate on behalf of those needing special support; gathers and compiles information and maintains accurate and complete records on community resources.
The Care Transitions Resource Specialist works closely and attends meetings with affiliated care transitions providers and hospitals.
Job Requirements:
- The successful candidate must have excellent communication skills.
- The ability to handle multiple priorities with attention to detail.
- Knowledge of benefits programs, housing, social services and other resources available to the aging population.
- Prior knowledge and experience with Medicare, MaineCare and benefits programs preferred.
- Passing score on Spectrum Generations Level II computer test (e-mail, basic Word, basic navigation of web for research, and entry into preset Excel spreadsheet.
Education Requirements:
- BS in Social Services, Gerontology or health related field.
- LSW, or ability to get conditional license, required.
- Equivalent combination of education and experience considered.
- Certified Information and Referral Specialist - Aging preferred.

Family Caregiver Specialist
Salary Grade: 8 ($29,432 to $41,204.80)
Hours: 32 per week
Location: Augusta, Maine
The purpose of this position is to develop and implement the Family Caregiver Support Program (FCSP) and Partners in Caring Respite Program (PIC) activities and services targeted to various populations in the area served by the Agency and expend effort to reach newly identified caregivers early in their care giving experience.
The individual responds to referrals to determine the need for Family Caregiver Support or PIC Service, provides information, training and counseling to consumers and caregivers, acts as a link to caregiver services and provides referrals as appropriate to a variety of assistance programs. The incumbent facilitates monthly caregiver support groups and also teaches SAVVY I and II caregiver classes per guidelines.
Job Requirements:
- Some knowledge of the legal requirements, standards, regulations, and laws applicable to programs for older and disabled adults.
- Strong grasp of gerontology, dementia care, senior's mental health, acquired brain injury and/or developmental disabilities as they relate to caregiver needs.
- Highly developed objective, subjective, and empathetic listening skills and ability to engage family caregivers in the service coordination process, and integrating caregivers into the care system.
- Ability to assess family dynamics and assist caregivers with communication skills and managing feelings.
Education Requirements:
- BS in Social Services or related field.
- LSW preferred, or able to obtain conditional LSW license.
- 3 - 4 years progressively more responsible work experience in community relations.
- 3 - 4 years experience in teamwork and liaison skills.
- Experience working in health care or in a non-profit organization highly desired.

HEAD CHEF
Salary Grade: 7 ($26,748 to $37,448.32)
Hours: 40 per week
Location: Hallowell, Maine
The purpose of this position is to prepare high quality, cost-effective meals on site for Meals on Wheels Program and congretate dining. Approximately 370 MOW's are prepared twice a week and 600 congregate diners are served weekly.
The incumbent oversees the day-to-day operations of food preparation and distribution of meals. The Head Chef is expected to develop an effective team of staff and volunteers. The incumbent is responsible for supervising the kitchen/wait employees and volunteer staff, and for tracking revenue and expenditures.
Job Requirements:
- Basic knowledge of personnel management as it relates to the selection, supervision, work schedules, motivation, preparing performance evaluations, and employee recognition and discipline.
- Basic knowledge of aging issues and nutritional needs specific to geriatrics.
- Considerable knowledge of principles, practices, materials, methods and equipment used in large scale kithen management.
- Working knowledge of food values and nutrition.
- Skilled in menu development within budgeted costs.
- General knowledge of therapeutic diets and food substitutions.
- Knowledge of meal pattern requirements and government standards related to nutrition.
- Outstanding food knowledge, culinary skills, food plating and presentation.
- Skill operating and basic troubleshooting equipment used to do the work (stoves, mixers, knives, dishwashers, etc.).
- Skilled in product ordering, inventory and cost controls.
- Strong experience in training culinary staff.
- Knowledge and ability to apply the principles of safe work practices.
- Ability to safely perform repetitive moderate to heavy lifting (up to 50 pounds).
- Ability to work long hours while standing, and under conditions of high temperatures.
- Ability to make independent judgments and decisions about a course of action to be taken within established laws, rules, policies, procedures, and guidelines.
- Ability to communicate orally, and in writing, with a variety of people, answering questions, and gathering/exchanging information.
- Ability to apply and explain rules, regulations, polices, and procedures.
- Ability to collect and organize information or data, and preparing reports.
Requirements:
- A.S. in Culinary Arts/Nutrition/Food Preparation or equivalent experience
- Experience equal to three years of full-time work in food preparation/food service/kitchen procedures in a commercial, military, or institutional setting.
- At least one year supervisory experience
- Certification in Food Sanitation desired.
- Have Serve Safe Certification, or be able to acquire within six months.
- Passing score on Level II computer test (e-mail, basic Word, basic navigation of web for research, and entry into preset Excel spreadsheet).
